Name Change

If you have changed your name because of a recent marriage, divorce, or legal name change, you can apply for a new passport.

If the name change occurred less than one year since your most recent passport was issued, submit the following:

  1. Form DS-5504 Application for a U.S. Passport, Name Change, Data Correction and Limited Passport Book Replacement.
  1. Your current passport.
  1. Original name change document (marriage license, divorce decree, legal name change document).
  1. One2”x2” (2×2 inches – 5x5cm) color photograph, full face looking forward, no plastic photos accepted, taken within the last 6 months with a light, plain background, size as shown above, and no smile.  You may not wear eyeglasses, sunglasses, hats, hair bands or anything on your head.
  1. You do not need to pay the passport or processing fees.

If the name change occurred more than one year since your most recent passport was issued, you should use form DS-82 and submit the following:

  1. Form DS-82. Passport Renewal Application for Eligible Individuals.  
  1. Your current passport
  1. Original name change document (marriage license, divorce decree, legal name change document)
  1. One2”x2” (2×2 inches – 5x5cm) color photograph, full face looking forward, no plastic photos accepted, taken within the last 6 months with a light, plain background, size as shown above, and no smile.  You may not wear eyeglasses, sunglasses, hats, hair bands or anything on your head..
  1. Applicable fee.

Form DS-82 Eligibility:  You can use form DS-82 only if:

You can submit your most recent passport
You were at least 16 years old when the passport was issued,
Your passport was issued less than 15 years ago;
Your passport is not damaged or mutilated.

If you are NOT eligible to use DS-82:

  1. Submit  form DS-11 Application for a U.S. Passport.  Do not sign the form.
  1. Your current passport.
  1. Original name change document (marriage license, divorce decree, legal name change document).
  1. One2”x2” (2×2 inches – 5x5cm) color photograph, full face looking forward, no plastic photos accepted, taken within the last 6 months with a light, plain background, size as shown above.  You may not wear eyeglasses, sunglasses, hats, hair bands or anything on your head.
  1. Applicable fee.

The two parent consent signature will apply in case of minors.  Passport applications for children under 16 years of age must comply with the following requirements:

  • Both parents’ signatures are required to execute the passport application.
  • The minor must also be present at the time of the passport application.
  • The minor’s original birth certificate showing the parents’ names must be presented for each renewal.  Both parents will have to present valid photo identification documents

Printing and Spelling Errors

Passports with printing or spelling errors caused by the U.S. government can be replaced at no charge at any time while the passport is still valid.  The requirements for replacement passports apply to both adults and minors.

Submit form DS-5504, your passport, a passport photo and documentation showing the correct information.

Passport applications for children under 16 years of age must comply with the following requirements:
